Insurance/Payment:



1. Do you take my insurance?
- We are participating providers for most major insurance companies. We strongly encourage you to contact your insurance company to not only verify we are a participating provider for your specific plan, but also verify your physical therapy benefits. You must inquire using our corporate name, Adel Physical Therapy, Inc. regardless of which location you choose.

2. Do I have to pay a co-pay for each visit or pay at the end of treatment?
- If your insurance plan requires a co-pay, it is due on the date of service. If after billing your insurance company we discover you have not met your deductible, this additional amount will be billed to you.

3. How much will it cost?
- You are responsible for your insurance plan deductible and co-pay. We encourage you to contact your insurance company before beginning treatment for specific information regarding your financial responsibility.
- If you do not carry health insurance, we do have private pay rates. Please contact our office for further information.

4. Do you accept credit cards? Which ones?
- Yes, we accept all major credit cards including Visa, MasterCard, Discover and American Express

5. Do I need a referral from my doctor?
- In the state of Iowa, you are eligible for physical therapy with or without a physician referral IF your insurance plans allows for it. If you are unsure, we encourage you to contact your insurance carrier. If your insurance does not require a physician referral, you can be seen by a physical therapist without going to your family doctor or medical specialist.
- If your insurance does require a physician referral, it must be from a medical doctor.
- ALL Medicare and Medicaid patients require a physician referral.

6. How long is the appointment?
- Each appointment is 45 minutes long. We ask that for your first appointment you arrive 15 minutes early to complete paperwork and provide insurance information.

7. How do I dress for my appointment?
- Dress in comfortable clothes.

8. How many sessions will I need?
- This can be different for each individual. At the first appointment, your physical therapist will evaluate your situation and after discussing with you, recommend a care plan. Factors that may affect this include the general health of the patient, the length of time they have had the condition, the order from the referring physician, etc.

9. How does physical therapy at your clinic differ from other physical therapy clinics?
- We provide one-to-one contact with the patient and never utilize support personnel to provide care to our patients. We use hands-on manual therapy; and rarely use machines or other passive modalities of physical therapy. We treat the whole patient by understanding their complete history rather than just treating their pain.

10. What can I expect at my first physical therapy session?
- At your first visit, a physical therapist will perform an evaluation and talk with you about your condition. The therapist then creates an individualized physical therapy program based on the findings of your evaluation. The therapist takes into account your expectations and goals, your age and physical abilities. The physical therapist will then use hands on techniques to improve joint, muscle and soft tissue mobility. After mobilizing the restricted tissues, the therapist will show you exercises to do at home that are specific to your problems. Following your first visit, your therapist will send a report to your physician regarding your evaluation and program plan. Before you return to your doctor, your therapist will send an updated report describing your progress in therapy.

11. Should I use heat or ice for pain and/or inflammation?
- This will depend on what you are trying to accomplish. Typically ice will help reduce inflammation. Heat on the other hand will help to stimulate circulation and blood flow to the problem area. Sometimes you have to try each to determine which is most effective given your condition. Your physical therapist will advise you on which method may be most appropriate, and how and when to use.

12. Do you treat women’s health issues?
- Yes, women often develop pain through injury, during pregnancy or after a surgical procedure. We will work with women to alleviate pain from many conditions including back, pelvic, muscle or joint pain. Physical therapy can help pain associated with sitting or intercourse; or numbness and tingling which could be related to muscle and joint dysfunction in the low back and/or pelvis. Physical therapy can also help with incontinence issues.

13. What do the credentials associated with different therapists mean?
- P.T. signifies licensed physical therapist
- MHS denotes a masters degree in Health Sciences
- OCS denotes Orthopedic Certified Specialist
- DPT denotes Doctor of Physical Therapy
- MS denotes Master of Science
- LAT denotes Licensed Athletic Trainer
- MPT denotes Master of Physical Therapy
As you can see, our physical therapists have focused their education within multiple specialties.

14. Where are you located? (Also see maps on “About Us” Page)
- We have three locations:

  1. Adel Physical Therapy is located at 516 Nile Kinnick Drive South, Suite B in Adel.

i. From the North (Dallas Center or Perry), go south on 169 thru the stop light; we are two blocks after the stop light on the west side of Nile Kinnick in a red brick building..
ii.From I-80, go north on 169; we are one block north of the high school on the west side of Nile Kinnick in a red brick building.
iii.From Highway 6 (Waukee), turn left (south) on Nile Kinnick; we are two blocks on the west side in a red brick building.

  2. Clark Physical Therapy is located at 2001 Westown Parkway, Suite 107 in West Des Moines

iv. From I-235, take 22nd Street North to Westown Parkway; turn right (east).
v. From the North, take NW 86th Street/22nd Street South to Westown Parkway and turn left (east).
vi. The entrance for Clark Physical Therapy is the same as the credit union.
vii. Proceed to the back of the 2001 building; we are located on the lower level in suite 107.
 
3. Clark “MFR” Physical Therapy is located at 1454 20th Street, Suite 109 in West Des Moines

viii. From I-235, take 22nd Street North to Westown Parkway; turn left (west). Turn left (south) on 30th Street (across from Holmes Murphy building). We are located in the West Winds Building (south of AAA). Proceed to the back of the building, suite #109, 1st door on the right.


15. Why do you require a photo ID?
- The Red Flag Rules put into law on 8/1/09 requires healthcare providers to identify, detect and respond to “red flags” regarding identity theft. It is for the protection of our patients’ identity that we require them to provide this proof.
